CYP2C9 PCR Assay
The Viennalab CYP2C9 mpx RealFast Assay detects the CYP2C9*2 (c.430C>T) and CYP2C9*3 (c.1075A>C) variants simultaneously by real-time PCR. Both variants reduce enzyme activity and cause wide inter-individual variation in the dose response to certain drugs, in particular coumarin-based anticoagulants. Identifying carriers of a poor metaboliser phenotype supports dosing during anticoagulation therapy with warfarin, acenocoumarol and phenprocoumon, where reduced enzyme activity carries a risk of severe bleeding or of therapeutic failure.

The Viennalab CYP2C9 mpx RealFast Assay determines the CYP2C9 genotype for the two most relevant reduced-function variants, CYP2C9*2 and CYP2C9*3, in a single real-time PCR run.
Clinical background
Both variants lower the activity of the CYP2C9 enzyme, which causes wide inter-individual variation in the dose response to drugs metabolised by this route. That variation is most pronounced for coumarin-based anticoagulants. Patients with low enzyme activity carry an increased risk of adverse drug reactions and of therapeutic failure.
Application
Detection of the poor metaboliser phenotype supports dosing during anticoagulation therapy with warfarin, acenocoumarol and phenprocoumon, where the consequence of overdosing is severe bleeding.
Method
The assay uses real-time PCR with the RealFast 2x mpx Probe Mix and the CYP2C9 mpx Assay Mix, which targets c.430C>T (CYP2C9*2) and c.1075A>C (CYP2C9*3). A *1*1 control and a *2*3 control are supplied. DNA isolated from whole peripheral blood, dried blood spots, buccal swabs or saliva can be used, and DNA extraction reagents are not supplied with the kit; ViennaLab recommends the Spin Micro DNA Extraction Kit (REF 2-020), ordered separately.
